2017/2018 Teacher Grant Application Form
Teachers thank you for your continued partnership with the PTA. It's that time of the year were the PTA would like to give back to you for all your hard work and dedication to our students. As like in the past we have allocated proceeds from our fundraising activities to help reimburse for some of the supplies you have purchased throughout the school year. Without your help we could not have a successful PTA and we sincerely thank and appreciate your continued support and effort for making Sessums Elementary a top tier school.
A few things to note in completing the application form.
- The Maximum reimbursement amount is $100.00 per teacher.
- You Must be a Current PTA Member (can be done online, thru the Stallion Shop).
- You Must provide copies of all receipts for purchases made.
- ALL Receipts MUST ONLY contain the items being reimbursed listed on it, otherwise application will be rejected and you will have to resubmit a new application form.
- The Deadline for submission is midnight (12pm) February 21, 2018. NO EXCEPTIONS
- Payments will be made by check during the School Week of March 19, 2018 (if we are able to it may be made sooner).
- Once an application form has been submitted, it cannot be amended and a new one will have to be completed. SO PLEASE COMPLETE CORRECTLY.
- All Grants are issued at the discretion of the PTA Board.
